Western Vale Cluster currently has around 32,000 patients served by 3 major practices working from 7 sites. The cluster covers a large geographical area including Cowbridge, Llantwit Major, St Athan and Rhoose. It stretches from Ogmore by Sea in the west to St Nicholas in the east, and the coast to the south and M4 motorway in the north. It has the largest geographical area of the 9 clusters. Aside from the urban towns, much of it is rural. There are some pockets of deprivation and a relatively aged population. These features represent a unique challenge when providing primary care to its population. The population in the area is set to expand due to housing developments and an increasing asylum population.

The Western Vale Cluster is in the process of developing its plan for 2024-25. It is based on the local population needs assessment, plus the emerging priorities identified by members of the cluster.

What are we working on?
We are developing projects to support the local population.
What we have already done
Western Vale has a Cluster Pharmacist, which has enabled GP’s to focus on patients with complex medical needs by undertaking annual medication reviews in a timely manner.
Western Vale has a high percentage of elderly patients, and therefore the enhanced care at home service has been fundamental in supporting patients to remain well at home. The early flu vaccination and patient checks on housebound patients has enabled high risk patients to be protected from seasonal influenza.
Future Priorities for the Cluster
We are also working on new initiatives to address some of the cluster emerging priorities. These include expanding the care at home service, supporting the increasing population, mental health, chronic disease and improvement in childhood immunisations.
