This form is designed for existing patients continuing their current contraception methods. If you're new to contraception, looking to switch methods or prefer a face-to-face consultation, please contact the surgery to schedule an appointment.
We aim to send your prescription to the pharmacy within 2 weeks. Blood pressure machines can be found in all surgery sites without the need for an appointment. Please note, that we are unable to issue combined contraception without up-to-date blood pressure and weight measurements.
