As part of our commitment to your ongoing health we will undertake an annual review with you if you have an ongoing medical problem. This review will be undertaken on an annual basis in the month of your birth and you will receive a letter or email to remind you and asking you to contact the surgery.

The review will normally consist of 2 stages:

1. The completion of any relevant condition(s) online review questionnaire, 

2. If required, the letter may also request you book a blood test and a face to face meeting with one of the Practice team, usually a specialist nurse or pharmacist.

It is important that you respond to the letter or email each year and complete this process so we can continue to monitor and improve your health. By completing the annual medication online review form it will allow us to review all your medication and update your repeat prescriptions for the upcoming year.

If required, contact the Practice to book a blood test.

If required, book a face to face appointment with the nurse specialist for diabetic or asthmatic reviews, or the Practice pharmacist for all other reviews either via the online appointment booking (My Health Online) or by contacting the Practice.
